All jokes aside, you need to get:

  1. Wirt’s other leg when the puppy takes you through Tristram. 100% missable, but you only need to get it once.
  2. Cain’s Cloak from the Haradric place in Act 1.
  3. Emporor’s Helm near the ruins of Caldeum.
  4. Meshif’s bottle after the appropriate time.
  5. Marshland Moccasins from Hawezar near the coast line
  6. Bear Paw Gloves from Scosglen Random bear drop
  7. Cowhide Chaps from killing the cows on the farms in Scosglen

You put them all on and open a town portal, you will end up in Modern Day San Francisco fighting a bunch of rainbow unicorns and clowns.

First WE need some endgame, because there is realy nothing to do at lvl 100 after 1 week.
