I was in the collections menu and the dialog for Malady of the Soul was finishing (after exiting the basement) and I got stuck in the Collections menu without the Codex of Power and Challenges visible there.
I managed to fix it by going to map and then ESC out of the menu, but I couldn’t just ESC out of the collections menu directly.
Malady of the Soul appeared to finish correctly.
