On the first quest step in the capstone dungeon at world tier 2, we’ve completely cleared out the accessible part of the dungeon where we are supposed to “Collect Animus from the Revenant Knights” and the progress bar is not filled all the way up. It’s sitting around 90%. There are no more enemies to kill and interacting the the Animus Urn and the locked door do nothing.

I have suffered the same bug in several dungeons. But for me it came to a head in the Cathedral of Light as I could not progress.
It seems that if you enter the room and kill an animus carrier too quickly, especially if you automatically get the animus instantly because it drops on you, it doesnt always register. This seems to be particularly acute if you have more than 1 animus carrier together.
The way around this is to do it all more slowly. Enter the room, wait a few seconds. Kill the animus carrier and if possible, wait a couple of seconds before picking up the animus. But the key thing seems to be if there are 2 animus carriers, kill one, let the animus register and wait a couple of seconds before getting the second one. Super annoying bug that led to me having to clear some dungeons several time. Not to mention the time it took going all over the dungeons to see if I was missing one before I realised what was happening.
