Aspect Of Untimely Death seems to apply to either a hit or a part of hits that make up the Blood Surge skill.
The video demonstrates what I also described here Notes on Overpower Blood Necro - #61 by Ashendar-2769 :
- Damage of the first part of the skill, the “draws health from the enemies” travels across the screen from bottom right to top left
- The initial stacks get used while the skill applies damage(buffed by Untimely Death) when travelling from bottom right to middle of the screen generating 10 stacks
- Second half of the screen applies damage consuming the 10 stacks and generating 6 more
- The first nova occurs and consumes the 6 stacks generated at point 3. Doesn’t generate stacks.
- The second nova occurs and generates 6 stacks.
The aspect itself reads :
Each percent of your Maximum Life you Heal beyond 100% grants you 0.5 bonus Overpower damage on your next Overpowering attack, up to a [20.0-60.0]% bonus.
Even if the Blood Surge was considered a 2 part attack(draw+nova), the current form as interpreted by Untimely Death is a 3 part attack without the Blood-Bathed aspect and a 4 part with Blood-Bathed aspect.

Extra Proof video demonstrating this as well as the interactions between Rathma’s Vigor and the Bloodbath legendary paragon node when it comes to Blood Surge.
Bloodbath does not consider the overpower proc from Rathma’s Vigor as guaranteed, it does instead ONLY consider the one from Paranormal Blood Surge.
Rathma’s Vigor is also wrongly interacting with Paranormal Blood Surge in combination with the Blood Bathed aspect, once the stacks of paranormal grant the overpower and are consumed, the echo nova can (and will consistently if there are still enemies around to get hit by it) consume Rathma’s Vigor as well.
