I found an issue with the Mirage quest. It’s actually possible to kill Jazadar so quickly that the quest completion is not awarded… And I did this at least 4 times before figuring out what was going on.
I had Razorplate equipped, which was giving me over 3,000 thorns damage. As soon as I broke the chains, Jazadar would be killed. His body wouldn’t even be visible before I just saw a corpse and a pile of coins on the ground. I used town portal to return to town, immediately use the TP to go back and the chains were back in tact. Break the chains, corpse and pile of coin. Repeat.
I thought maybe my minions were killing him, so I sacrificed my minions. Nope, still just spawns a corpse and money. Then I unequipped Razorplate. Now I was able to see the enemy, kill him, and be granted the completion of the quest.

So glad I saw this… Literally just ran into this situation, I was so confused but I had razorplate on as well…

Having a similar problem with a minion necro. Basically got stuck at the kill the J-guy stage. left and came back after various experiments. Finally took the summon skeletons off my bar - so only had the golem available. The I directed the golem to smash the chains. And the quest was completed at that point.

This is an odd glitch. I thought I had already taken razorplate off, but I hadn’t. When I reloaded and unequipped, it worked perfectly.

Happened to me 5 Minutes ago too.
Still happening … just have it here too … I can’t break the Chains … the quest doens’t go after that
I just re-logged the game in front of it and it worked
I just came across this problem as well. Sure enough taking the Razorplate off solved it. Odd because it didn’t seem like the mob was spawning at all. I broke the chains and then nothing happened.
Yup same thing still going on months later. Thank you for the post this was the only valid workaround.
Can confirm, issue is still going on as of 3/28/2025. Kill the J-guy, get “Return to the Altar”, and… nothing happens.
Drop the quest and pick it up? Nothing.
Drop the quest, log off and back into the character, pick up quest again? Nothing.
