On my druid, when I save a loadout in the armory if I have spent more than 70 skill points the equip button is greyed out. Tested all variations of gear/runes/Paragon/skills etc and the only thing that consistently disabled the equip button on a saved loadout is when I went from 12 points left to 11… IE if I spent my 71st skill point (or more).
