Similar to the PC bugs (there are more than these 2 reports):
-Altar of Ruin stronghold
-Stronghold Altar of Ruin incompletable bug
I also experienced it on console (PS5), where I had all 3 keystones but only maximum of 2 could be set.
-All enemies are defeated.
-Porting to town & returning didn’t help.

Same experience on my end, just tried it.
Edit: as I was walking out I saw the keystone in the spawn location :). I died after having collected the first keystone. I guess it moved to the spawn location; weird behavior.
Same here, its like there is an invisible wall and I can’t even walk into the Altar.
This bug still exists in 2024.
