It’s a small rectangle on some items. It’s half the height of the little box and ¼ of the width. Most of the time it’s dark grey and in the bottom right corner, but sometimes it’s black and sometimes near the left edge. It’s only on chests and pants and I’d estimate only on 10-20% of them. Also appears to not affect anything, I’m just concerned it may be part of a larger problem as I’ve never seen this before.
