I’m on an Alienware PC which I know can handle most issues on lag, but my character isn’t even responding on basic attacks like bash. I can move and pick up herbs and other items but attacking with right & left mouse is a no go. I’ve tested the mouse and even switched it out. It’s not my equipment, something is going on in the background,

Options > Controls > check boxes under keybind presets // and move & interact buttons?
did you change any of these?
you could always back up your Localprefs.txt file by renaming it to Localprefs.old.
The game will make a new one and see if your basic skills on mouse work.
It is the same on Xbox series x also. Basic is inoperable unless you have fury. Acts just like a core skill. Happened after I reached level 94. This being said it is not your settings. It is a bug.
Do you have the Shard of Verathiel equipped? That would explain why it is happening to you.
