I just looked through all of my titles, and don’t have efficient yet. So checked all of my incomplete achievements, and found nothing. None of them reward the Efficient title. Looked online, and the only site listing Efficient had no info on how to get it.

The Efficient prefix is one of the ~30 new prefix titles added with the S4/1.4 patch but does not seem to have an associated achievement or activity tied to it yet. It’s likely something for future content, as this has happened many times with previous patches, where the titles end up in the game before the actual content to acquire them does. Same thing happened with the gauntlet titles as well.
I’ve never seen this amount of “unobtainable” titles added in one go though. Usually it’s only a handful.
