I have cleared 50ish nightmare dungeons and pits on torment IV just alone today and not a single alter.
Yeah, It took a while for me as well. RNG is what it is.
Options are to keep doing it, or ask somebody in trade chat for them (you may have to pay though).
There isn’t anything that can be done about it. (If your not lucky enough to find it just yet)
Make sure to be using the potion, and really got to be doing it on t4.
Think I found 1 in a Roothold,1 in a pit and 2 in nightmare dungeons.

Yep, they are rare. Up to 3 out of 4 myself, and didn’t find most of them until hitting T4. One in a pit and two in nightmare dungeons.
Yep, they are rare even with the potion. I’ve gotten 4 so far in NMD’s nowhere else. But I went the entirety of season 2 and never got any of the unique ones of the vamp powers. I played a ton of hours in S2, and far fewer hours S5 6 and 7.
I got pretty lucky. Found about 5 in nightmare dungeons, a few in the pit and a few in the rootholds
hoarfrost dungeon, takes a minute to clear, pop a pot, took me about a hour to find all 4 altars.
Don’t know if I am lucky but missed a lot of them but when i made a new char i got the rest of the witch powers. Just did a few quests and then I could buy the ones i did not have. Even the legendary ones.
there are only 4 witch powers you unlock through altars. most of them are purchased.
