Didn’t she create Sanctuary as a respite from conflict (hence the name)? Basically her and Inarius got tired of fighting. So why does her return “usher in an age of darkness and misery”?

Because she is evil. One of the henchmen. She has always been part of it.
Will probably get some insights form the new Artbook (always good read through into lore) and obviously once the game comes out.
Could it be that the Nephalem (basically the rebirth of power of Sanctuary) have caused her to turn against the creation? Seems like a plausible story arc.
How does this work…we killed her in D2…

Unless you trap the souls Demons and Angels are eventually reborn through their respective origins (I could grab the book, Spine of Tez… and Wings of…).
I’m fuzzy on if we killed her in D2 but if we did and never trapped the soul it doesn’t matter.

If you’re talking about the Lilith in Diablo 2, that version was apart of the Pandemonium Event for Diablo 2, not exactly apart of the game’s story. The sin war trilogy revised Lilith’s origin and fate (for example in Diablo 2, she was considered Andariel’s mother, but this was changed). But pretty much she was sealed away before the events of Diablo 1.

So they rewrote her origins…in a book. But that’s not how it played out in the game. I didn’t read the books I followed the storyline in the game. Deckard Cain died in the game does that mean they can now rewrite his death so he didn’t die from butterflies?

There’s a difference.
Cain’s death happened as apart of the campaign. Lilith’s addition in Diablo 2 was an event, not exactly considered canon. The Sins trilogy revised her origin, which is also summarized in some of the journals found in Diablo 3 RoS.

Because it’s a recognizable female. The antagonist of the New Wow Expansion is also a female. And I don’t mind because they aren’t doing silly things like giving Diablo Boobs like D3 did.

i too am curious like the OP. If she helped create nephalim and santuary, then why is she the villian? also our heroes are still nephalim or what? not much of a lore person here any help?

Lilith wanted to use nephilim/humanity as a weapon. She also represents the demonic/evil part of humanity. Lilith is the daughter of Mephisto, the Lord of Hatred so she is literally the daughter of hatred.

Her lore books that drop in Pandamonium Fortress explain it.
She represents all evil aspects of humans, something we can all relate to in real life, including lust.
That’s how I see it too.
If you collected and listened to the Books, The Writings of Lilith, found in Pandemonium of Act V, then you know she used Inarius to create an army to win in the Eternal Conflict.
In Part 4 of her Journals she says “Inarius believes that escape is enough. In time, I will show him that even victory is possible. But first, I will give him children.”

The real question is… can we side with Lilith? 
I mean, hey, she did need the blood of the willing, to be freed!

Considering that Diablo name is at the game title, Diablo will come and kill Lilith for us and he will be your typical final boss.
I’m sure she appreciated their sacrifice. 
Not so sure it would bode well for players of Hardcore Characters. 
(As in discarding those who helped her… rather than rewarding them).
