I personally rolled 500 Mojos upgraded from rares in the cube and have not seen one yet. I also decided to run a couple ancient vaults and a few normal rifts at t16 and have not seen this item. There are a few other reports as well of similar findings.

Likewise. I upgraded over 300 Rares, did a full round of T16 Bounties, ran 10 GR 90s and no drop. Even though it shows in the cube, I don’t think it’s in the loot pool yet.

I even ran a bounty to see if it was a Blacksmith crafting item that you had to do a bounty to get a recipe - No there either.
Server has shutdown - I imagine to add the missing items and problematic issues not included in the last patch,
I hope the shutdown was to tweak further numbers seeing how VERY BAD the last numbers were to several items/builds.
