There is a bug, when you do a big nephalem rift and you die, you cant respawn. The Screen for resapwn in the city, at the place or at a save-point doestn´t pop up. You just chill there with your tombstone and see how your time is running out.

same happened in a regular rift while i was leveling fighting final boss
