I’ve died twice now without a proc. Anyone else notice this?
Most likely, the Templar was ‘dead’ when you took a killing blow. While followers are dead, all of their skills are paused until they revive.

Also emanating powers don’t function while the follower is down.
It is always important to either use the immortality item or stack a lot survivability to the follower when using the all skills or cooldown reduction item.
