The recipe would be:
1x Maddening Questions
1x Never Ending Questions
1x Yet More Questions
1x Legendary / Ancient Legendary / Primal Ancient Legendary
The end result would be the item in the cube is either upgraded to a Primal Ancient Legendary or if already a Primal Ancient, it will re-roll but keeping it Primal Ancient.
This would give new relevance to Set Dungeons as the tomes used in the recipe are only found in set dungeons. There’s a bit of RNG to acquiring them, so it shouldn’t be a recipe that is too powerful by making Primal Ancients too easy to attain/re-roll.

No because i don’t like set dungeons. 
I can Master the Immortal King dungeon in around 80-90 seconds per run.
And for that I’d get to turn all my gear primal?
That’s a hard pass from me.
I think people are way too obsessed about primals.
That’s a “no” as my answer, in case you’re wondering.
It’d be too easy to target the nearly impossible primals which, for me, are weapons and bracers. Since I have never gotten one (bracer, specifically) it would be a near-automatic upgrade.
And yes, many sane people would just run Immortal King over and over. It is by far the easiest and quickest.
