This so far has only happened to me in season 25, and is the 5th time so far it has happened (and I’ve witnessed it happen to one other).
I die in a rift (cheat deaths apparently not doing anything randomly) and no menu appears to ask me to revive at corpse, revive at last checkpoint, etc. I can’t do anything except leave a game. I’ve seen this happen to another person and even clicking their gravestone to revive them does not work.
