Many times i see my Simulacrum go away (with Haunted Vision on) and most of the times i die right after since i don’t have the thoughness. Today i got it in a clip where it happends 2 times in a short time.
Do i do something wrong or is it a bug?

Simulacrum/Self Sacrifice:
If you would die with a Simulacrum active, instead the Simulacrum is destroyed and you are fully healed.

I play on the Xbox console. I have been in Multiple high Level solo GR’s and had the same thing happen to me. My simulacrums were killed from direct shots the rift guardians, elites and champions however I didn’t die. I have had to hit simulacrum multiple times in the middle of big mobs as well. Not sure if it’s a bug or what, but the sims do take damage which goes against what the set is supposed to do. I just failed my 127, Raizel one shot my sims twice in a row, all the whole I didn’t die.
This is not a bug. Read the description on the set, then read the description of the runes for Simulacrum.
If that does not answer your question, then I suggest you scroll down through the bug report forum and read some of the many other false bug reports on this, since it has been answered many, many times already. In fact, it was answered in the post right above yours.
