When the Witch Doctor skill Spirit Barrage: Phantasm is cast during Spirit Walk, the Phantasm won’t actually spawn until Spirit Walk ends.
In the following videos I demonstrate the bug, and how the skill usually works - in both cases the phantasm attacks 10 times over 5 seconds, but in the first and bugged video the attacks start only when Spirit Walk ends:
