I’ve beat the Dark Lord about six times now and not once has the Red Soul Shard dropped. The first time I beat him with a new character starting at level 1. Then I beat it a few times with the same character and then switched to my other saved ones but the gem still hasn’t dropped. What gives?
You can only have one drop if you don’t already have one. Just like every other legendary gem.

Bear in mind that if you want more than one of these gems for some reason, you can force further drops of it. What determines whether the end boss drops it or not is whether the game can “see” one already in one of these areas…
- Your hero’s equipment
- Your follower’s equipment
- Your stash contents
So, switch to / create another hero. Take the gem out of your stash into that hero’s inventory. Switch back to your main hero. Run the dungeon again, and you’ll get the gem again.
