When I drop a primal ancient, no matter if a world random drop or dropped out of my inventory, there is NO pentagram showing on the minimap.
Does it only show under certain circumstances? Or EVERYTIME? Why can’t I see it then?
Mini-map icons are only displayed when the item initially drops. Once you’ve picked the item up, dropping it on the ground again will not create an icon on the mini-map.
This is not a bug.

As I said above there also isn’t a pentagram on a world random drop (or initial as you call it)… Dropped 4 or 5 primal ancients so far, not one showed the pentagram on the map.
Were you in a party? If another person dropped it for you, you will also not get an icon on the minimap.
