Simulacrum is supposed to reset its cooldown when you die. Technically, “Final Service” is death, it has its own cooldown as well, though simulacrum is not reset when final service is used. Is it intentional? Because if not, then it is a bug.

It is intentional. The wording says “upon death”. If it also was supposed to reset when you proc, it would say “when taking fatal damage”.
If your final service procs it does not reset the cooldown but it still kills the simalcrum which means when you die again. The cooldown is not reset.
Simulacrum dying when Final Service procs is a bug, but it is not what was reported here.
