Can’t find this guys helm in the transmogs list on D3 Planner. Anyone know what it is. It looks like an in game item.

I don’t know the helm, but the shoulders (with horns) are Vile Ward.
EDIT: Found the helm. It’s Hellscape Mask: Hellscape Mask - Game Guide - Diablo III Only looks like that on Crusader.
yea, hellscape mask. I wonder if you can get it in game on a wizard. That would be the perfect mask for my setup. (Edit: nvm.)
