Slow down a bit… let him time to fully spawn before killing him… same idea as those Gelatinous goblins.

Azmodan, In for the Kill

This gets reported each season on the regular bug forums. It is caused by killing him too quickly. Let him fully spawn in and make sure he is done talking, then let him make the first move before attacking him. This usually works, but you can also try reducing your damage so it takes more than one hit to kill him. Most people do not encounter this because they are not strong enough to one shot him when they kill him this early in the game.
