then went into Reforge clicking away until Primal on the item.

now i’ve had items disappear before when converting by set but ive never had it happen this way.
People please do not have anything in the cube when you go to change page tabs.
A few things:
Using recipe #4 of the cube will never result in an ancient item. It says as much in the description of the recipe.
On PC, the item never leaves your inventory, so there is no way for it to be deleted.

factually wrong on that update I used to roll for ancients and succeed all the time.

No. You never did. Not with the Skill of Nilfur Recipe (Convert Set Item). It even says so on Kanai’s Cube Recipe. And always has:
See the statement in red text: Resulting item cannot be Ancient
However, Reforge items (Law of Kulle) CAN be Ancient or Primal.
As far as deleting items from the Cube. When changing Cube Pages, yes, they are removed from the Cube panel. But, NOT from your Inventory.
