I’m really sorry if this is common knowledge but I was struggling for materials and forgotten souls then searched for a way to help me out.
I hit jackpot. It’s incredibly easy and again, I’m sorry if this is common knowledge but here goes:
Upgrade Polearms to get the Bovine Bardiche, stick that in the Kanai’s Cube and it gives you a portal to the Cow Level. The cow level is full of hundreds of mobs which all have a chance of creating the Vision of Enmity (the loot HEAVEN) which jams you full of crafting materials, legendaries and, well everything! I just got two Ethereals in one portal.
Best thing about this is that you can complete the Vision of Enmity and return to town, then take your red portal back to the cow level to continue grinding cows for another Vision to spawn!
I’ve used one Bovine Bardiche and so far had 3 portals spawn and I’m not even finished on the Cow Level.
Just incase people didnt know about this - give it a go, it’s fantastic. Happy Wednesday!
To REALLY maximize this, have your follower equip the sets you can craft to enable double drops on Deaths Breath and the other set to increase chance of getting Greater Rift Keystones and you’ll be grinding out thousands of Deaths Breaths in not even 1 hour 
The sets double up with the Altar of Rites so you’re going to be earning so much Deaths Breath, you literally will not be able to spend it all XD

Your tip about Bovine Bardiches is not new but fine to bring up especially for classes that rarely see polearm drops (I believe Crusader is the best class to use for getting Bovines from the cube). Early in season it could be wise to use crafting mats sparingly and wisely.
Also getting visions to spawn from some of the crowded maps is also relatively easy.

Quick update - this method got me 5 Ethereals from 1 Bovine Bardiche and multiple puzzle rings!
Correct. The Crusader only has 3 Legendary Polearms in their Loot Table. And one of them is the Bovine Bardiche.

Vidian’s temple is also very good and doesn’t need another item to spawn it. I typically get 3-4 visions per run on average.

Visions are the best total.

Just remember that nothing is guaranteed. We can run several cow levels and have mediocre results (as with any activity in the game).
I ran a cow level yesterday. Got four visions. Two of them shallow (only one level and two levels deep). Two deeper with lots of drops - none ethereal (not that I expected any of course). Remember that it’s a good idea to have a Broken Crown on follower and a gem in your own helmet if you want extra gems here in the beginning of season).

Visions are a bit ridiculous, especially with how often they show up - my last one gave me 3 inventories full of legendaries, including 3 ethereals. Of course, when I tried the cow level technique, the “fulminated into reality” announcement popped but there was no portal to be found anywhere, so it was a bit of a waste.
It didn’t show on the map (on “m” press)? And you didn’t get a yellow arrow?
There is also a small chance that a vision portal will spawn right under another portal making it hard to see, or click.

Those were the first things I looked for (since I remembered seeing them every other time), and they were nowhere to be found - no portal icon, no “ping” indicator, no yellow arrow, no nothing.
The bonus for Keystone drops only applies to Nephalim rifts. It won’t do anything to improve drops in Cow level or Visions.

Worked for me last season in visions. Haven’t equipped follower with Cain’s yet this season.

Monk’s not a bad second for BBs. At times last season they felt self-sustaining in terms of drops in NtCL and fissures. Plus I6R4 monk is close to the fastest T16 build in the game.

I should have pointed out that I meant if your main class is one that doesn’t see Bovines in drops it could make sense to make a level 1 crusader that is also useful for rolling and crafting strength gear for templar follower.

The only problem is that it’s a very long (and highly linear) area, so each time you finish a Vision, you need to run through half of the level again to get back to where you left off.
If only the Vision would dump you back at the original location of the entry portal instead of in Town (unless it only does that if you used a Town Portal inside the Vision in order to empty your inventory of garbage Legendaries)…
Me, and many others were mentioning this already on PTR forums when they introduced visions. There was not even a response from Blizz…
It is the way it is now…

yes, that is true. If you have a mobile character, you can traverse back through the visions to the starting point in vidian’s temple (depends on length of the visons of course).
