This is NOT a bug.
To add to the link that @huertgenwald provided you, the video below shows what to look for. (It’s from Season 16, but it still spawns and can be done in Adventure Mode).

Many thanks for your input, maybe you should concider giving number around very rare ? After 200 spawns for a 5% should have spawn 10 times, let’s say i’m very unlucky yet it should have spawn
Blizzard doesn’t publish the chance of these types of spawns. Most of the percentages that you see (such as the chance of finding a primal or a Ramaladni’s Gift) were worked out by players over time.
Some achievement spawns (like certain unique areas or monsters) can be very rare. It may take you hundreds of runs, or you may get lucky and find it quickly. In any case, bad luck is not a bug.

Got it yesterday, it is just rare. Like the last 4 ultra rares I need for Irreplaceable You that I have not seen is about 4 seasons.
It finally showed up for me 2 days ago. I took a picture and a screenshot for proof. LoL
After all of these years of playing it on Xbox One I now see it in Aug 2024.
