In S25 on my Frenzy Savage barb and IK sets (all I have tested) when I equip a soul shard in the helm slot my CDR is bugged to 3.25%.
I checked this on my wife’s account and on my DH, neither have the same issue.
I have created a screenshot that has the CDR shown with the helm slotted with no gem, a royal diamond, and a rank 3 sliver of terror, but for some reason I can’t attach it to the report.
Paragon points for CDR is maxed.
CDR with no gem - 10%
CDR with Royal Diamond - 20.79%
CDR with Sliver of Terror (rank 3) - 3.25%
Sliver of Terror
- Base Power: Your cooldowns are increased by 25%. For every skill on cooldown, you take 12.5% reduced damage and deal 12.5% increased damage.
Have you forgotten to account for the Base Power of the Sliver of Terror ???

sliver of terror increase ur cooldown not cooldown reduction 
It seems some people are constantly ignoring, whether accidentally or deliberately, the negative effects some items have.
