Forum Avatars - Missing the small icon for the default avatar

There are posters which have the default avatar, i.e. the Diablo face. The underlying image is this…

Whenever someone quotes one of those posters, the smaller icon that would normally be shown next to the quoted section shows as a broken link to me. The underlying image is this…

That second image link gives a 404 Not Found error.

Not sure when this started, but it’s at least a few days.
So, did something server side get accidentally deleted?

This error is minimum a month old.

That’s also broken / 404.
