From what I am reading on Forums it sounds like its nearly impossible to build toughness high enough to allow the other never used tokens on high GRs.
Would be fantastic to be able to use the other 2 tokens.
Can followers have a built in 90% toughness?
I am sure there are other suggestions better than this.

Sadly yes. The Immortal Token is a necessity that it dwarfed other 2 tokens.
It’s been a while, but I think your follower starts insta-dying around T6.
You haven’t done much reading then. There is literally a thread called ‘FOLLOWER TOUGHNESS - VIDEO INCLUDED’
Thanks for the heads up. I guess I’ll try equipping my follower for survival in the next season to see how it goes.
Only the defensive legendary gems (mutilation and esoteric) work on followers?
As of right now, it seems that way yes. Things may still change, this is not conclusive.
Ok, it’s just when I search online all sources say they don’t work, but they might mean that the gems powers don’t work on your character if the follower has them equipped.
