I did 15 runs with my UE MS seasonal character - lots of cursed chests, which ruin your chances of the mysterious chest spawning. Unusual rate of cursed chest spawns, and several games where no chest spawned at all, which is very weird from my experience. In the past, typically at least 1 chest has spawned on the level. I even had 1 game with 2 cursed chests AND 2 normal chests. I have NEVER see this happen before, in thousands and thousands of gametime.
Anyway, there was a trick with the mysterious chest for the ghoul’s blade back in the day - switch to campaign mode. I fired up my trust s6 impale non-seasonal SC character (it’s the only 1 of my characters that are showing campaign mode as complete, which is ridiculous!). 15 runs on the dot and it spawned. No cursed chests in campaign mode btw, so it increases your spawn rate chances right off the bat! Here’s the video to prove it:

so it is still in the game, on PS4. 5000 runs my rear end. Stop spreading BS.
1st run: only couple of weapon racks
2nd run: two chests, a lot of weapon racks
3rd run: one chest, few weapon racks
4th run: cursed chest in confirmed spot, few weapon racks
5th run: one chest, few weapon racks
6th run: weapon racks… a lot of them
7th run: one chest, one cursed chest, few weapon racks
8th run: weapon racks… a lot of them
9th run: weapon racks… a lot of them
10th run: two weapon racks, not in chest spots though
I normally play on PC, but I gave it a whirl on PlayStation for “science.” RNG was kind to me since I found the mysterious chest on my first try on my seasonal toon on T16 in adventure mode. FYI, the Falcon Wings plans didn’t drop when I opened the chest because I’ve already learned it.
there you go. My brain filters them out lol. They never drop anything good, so why waste time with them. Too many hours playing D3 and the brain filtering lol.
There you go. Can you do a few more runs for science and see if you notice any chest weirdness spawning? I noticed unusual spawning imho - very high proliferation of cursed chests, more than it ever used to be, and several runs with zero chests. Having done a lot of runs in the past for this chest (mostly helping other people), this doesn’t reflect on my recall of what it was like in the past.
Science experiment number two complete and I’m far short of 5000 runs through the area. LOL This time it took me 8 runs to find the mysterious chest a second time. Side note – I found a rainbow goblin on run number 7 but still no Cosmic Wings!
I didn’t mention it in my last post but I should have. There was a cursed chest on the floor when I found it the first time. This set of 8 runs had cursed chest(s) each time except the last run when I found the mysterious chest.

Edit: I forgot to mention, I’ve been fully exploring the floor each time. It’s very easy and fast to do using the DH’s GoD set. Zoom! Zoom! Zoom!
I’ve been fully exploring the level too. Did only 16 runs for no mysterious chest. 2 runs without any chests whatsoever. 1 run with the 2 cursed and 2 normal chests, which is very odd. That’s why I said I was seeing chest oddness on the level and was curious if you were seeing similar or not.
