Tyrael tells us that destroying the black soulstone will permanently kill Diablo. Malthael destroys it and sucks in it’s power, but Diablo’s essence is now loose? Like what?
he was the reaper of souls and when he broke it he absorbed diablos soul but when we killed malth every soul he took was freed so in essense we unleashed diablo upon the world again
Alternatively, Tyrael is secretly working for Diablo. Everything he does seem to be a disaster.

Destroying the Black Soulstone was never guaranteed to kill Diablo or the other great evils trapped within. The idea was proposed by Adria as a way of having us help her gather the rest of the great evils essences within the Black Soulstone under the guise of destroying it, only after careful preparations were made. However in truth, we were only collecting the essences to further Diablo’s and Adria’s plan to revive Diablo as the Prime Evil. Since Adria had lied about the reasoning for collecting the 7 great evils essences, it’s also likely that she had lied about knowing a method to properly destroy the Black Soulstone along with the Demons it contained.
Thus far, the only effecient way of ridding the great demons is to contain their essence/spirit within angel-made soulstones and destroy said soulstone upon the Anvil of Destruction Hellforge within the Burning Hells, as anything destroyed their is supposedly negated forever, and thus the demons would be forever sent and trapped within the Black Abyss (unless their essences been marked). Since the Black Soulstone was made by human means, it wasn’t known whether destroying it upon the Hellforge would result in the same effect or simply revived Diablo, as such the High Heavens did not choose to destroy it while it remained within the Heavens.
Since Malthael later took the Black Soulstone and later destroyed it in order to absorb the trapped Great Evils essences within himself, his death caused the Release of Diablo, as well as the other great evils who were still trapped within Diablo, although in time the other great evils managed to separate themselves from Diablo and thus becoming free (hence why we see Andariel and Duriel).
So long story short, simply destroying the Black Soulstone was never going to permanently kill Diablo or any of the great Evils.

yeah lore wise the heroes did just that but they retconned it to where the primes all survived and set in motion the events for D3
because when you shattered the soulstone it dropped a variety of assorted flawless quality gems
