Cleared the entire level and there’s no sign of anyway to a new level. I’m on campaign mode.

On Pandemonium Fortress Level 2, look for the section that has all the Portals.
Enter the Portals until you find the entrance to Pandemonium Fortress Level 3. (You may have missed the entrance to Level 3 if you skipped going to the end of a path that looked like a dead-end).
As you probably already know, Level 3 is just a straight path to the entrance of the Heart of the Fortress, and… Malthael.

In campaign, after you you break the soul prison chain and go through the first portal, go north-west as soon as you can, then take the first left. Go as far to the left as you can until you can’t go any further, then start to go up. Then you’ll hit the series of portals that willlead you to the door to Pandemonium 3.

The location of the first left will vary, sometimes it’s farther along and sometimes it’s closer. Here’s an alternate one, with a closer first left then a longer run after you go left:

It’s always northwest then the first left and go as far left as you can and then go north again through the series of portals that will lead you to Pandemonium 3. You’ll always find it if you take this path, it will always be a variant of northwest then left then up again.
