In season 25 my DH has ancient gloves (Theo & Tasker) and an ancient ring (ring of oclusion) which will not upgrade with caldesan’s desire in the cube. I have all of the required mats in the cube together with the ancient item and so I’m not sure why I always receive the message “transformation failed.” I have already upgraded all of my other ancient gear using the cube, sometimes with the regular ancient gem and sometimes with an upgraded (3 upgrades) soul shard. Can you help me with this?
Not aware of any bug related to this, so I’m thinking the ingredients are probably not exactly right. The items are actually ancient right? And your using 3 max level normal gems? Not diamonds?
Could you tell us exactly what you put in the cube that fails? Ideally with screen shots in case there’s something you didn’t think about.

Your Seasonal Demon Hunter, Vesper, has ALL Ancient items equipped. No problem there.
All your items, except the Tasker and Theo and the Oculus Ring, have been augmented with Rank 3 Soul Shards. You used 3 Flawless Royal Emeralds for Dexterity on all the items you augmented except the Obsidian Ring of the Zodiac, with which you used 3 Flawless Royal Amethysts for Vitality.
I can only assume, as @Cascade stated, that you may be forgetting to add the Flawless Royal Gems or Legendary Gem/Soul Shard when trying to augment the Gloves and Ring.
Also, as @Cascade stated, you should post a screenshot of the items in your Cube, just before you click the Transmute button, in case it continues to fail.

I could include screenshots as suggested, but perhaps you can take my word for this that YES three flawless royal gems are in the cube, together with an ancient gem or a triple buffed soul shard. Perhaps I will go ahead and do as you ask, but I know that all of the ingredients are in the cube (I have done this process already with all of my other gear) together with the ancient legendary ring or ancient legendary gloves. Perhaps having the screen shot will you with your documentation of the problem even if doesn´t correct it.

The screenshot will confirm this, but the recipe won’t work with Flawless Royal Diamonds.

That’s not how debugging works. It’s not a matter of not trusting, we are trying to help.
Either there is a bug, or you’re not putting in the right ingredients. We don’t know which and neither do you. We all make mistakes without realising all the time, including me. We want to help you find the problem, and screen shots allow us to do that. If ingredients are wrong, we can tell you and your problem will be solved. If they are right and still not work, then we can take the bug more seriously.
