The double primal drop from fully unlocked altar does not work on bounty caches.
Actually not sure if that is a bug (as I understand that caches might follow their own rules) but it is a primal drop nevertheless so it definitely feel like one.
This IS NOT a bug.
Challenge Rift Caches and Bounty Caches have fixed loot. They have not, do not and never will drop bonus loot from any Seasonal buff.

Correct, that’s also why whenever we have a double bounty bonus, it drops 2 caches instead of having the double amount in it.

You’re wrong. I got double primal ancient drop from one T16 bounty cache. Also Kadala bloodshards trade gives double primal ancients too.
The only thing that does not give double primal ancient items is upgrading yellow item to legendary in cube - i was making some puzzle ring and accidently made primal ancient rogar huge stone ring, second primal ancient ring one never appeared in inventory.
Are you saying, that when you gambled one item from Kadala, and it happened to be primal, there was automatically a second primal in your inventory? Or did you gamble two items which luckily happened to be primal? That makes a difference.
Yes. Gambled for one item, it happend to be Primal Ancient, and then second primal ancient of the same item type (chest armor for example) was just magically put in my inventory. Today i got 2 PA chest armor from her.

I believe this is how it is intended to work. It worked like this on the PTR with Kadala giving double Primals for the price of a single item.
It does not seem to give double primals when crafting or from Horadric Caches. I can only assume this is also intended, but who knows.
I did not receive a second from my T16 bounty cache. Same as OP.

When you get another primal from a cache, I’ll bet you don’t get an extra one then either.
Can confirm a double primal drop (pants for my DH) from Kadala and only a single drop of a primal from a bounty cache.
It threw me for a sec, but figured it was on purpose.
