i hva just transmuted my ancient weapon to primal, after getting a primal… i cant seem to equip the item to my character.

Only one primal that was crafted using recipe Kanai cube recipe #11 can be equipped.
I bet you have another crafted primal already equipped on that character.
It doesn’t help that the cube recipe says nothing about this restriction - apparently, the Season 28 announcement (from almost a year and a half ago) mentioned it when the recipe was first added, but it’s never been mentioned since.
Actually, the Primordial Ashes feature and the new cube recipe for Primal item were added in Patch 2.7.5 . It is independent from Seasons – which is why the recipe works outside of seasons and cube-created primal items don’t go Poof when the Season ends.
Should a new season announcement include a reminder of the mechanic for primal items created with the cube? And what about all the changes that were made in Patch 2.4.3? And what about the changes made in patch 2.0.1? Where does one stop?
Fortunately, such discussions do not belong in the Bug Report category.
I’m not sure what that has to do with anything - my point was that the Primal Cube recipe restriction isn’t mentioned anywhere in-game, which means that anybody who wasn’t playing back when the feature was first added (myself included, which is why I mistakenly attributed it to a Season rather than a Patch) would have no way of knowing that restriction without looking back through previous patch notes.
It was asked for during the season PTR, essentially the earliest possible, and repeatedly up to the point that the game was retired.
If Bliz didn’t make the effort while actively developing the game, there’s virtually no chance now.

Well, there are a lot of things about the game that aren’t written down somewhere in game… and yes, there is no way of knowing about it unless you read about it “somewhere”. Some of those are bugs… some are quirks experienced by some but not all players, and others are actual game mechanics. Spending time on this forum is one way of learning about some of it. Now that you know about the restriction about the cube recipe for primals… I hope that you will take it in consideration before cubing a legendary item and upgrading it to primal.
There is a Game Guide but it hasn’t been updated for many years… and some of the info it contains is now outdated… and/or wrong.
There’s no way all that information about exceptions or restrictions could be fitted in the game… as others have said above, some of the regulars did try very, very hard to have Blizzard put in some info on the Cube recipe… because they expected the exact situation that we have now… to no avail.
So… in many more words that I wanted to write… that is what I meant.
I just got the same issue, i saw the ladder players with bot primal in hands, and i coudlnt wear up mine, but yes they are from cube recipe 11, so thats the answer as they say here
If you enable “Rarity Icons for Items” under Options/Gameplay, you can quickly see which items are “natural” primal and which ones are “upgraded” (in the cube) primal. For example (click on screenshot to zoom in):
This player is wearing 3 primal items: the weapon, the pants and the amulet. The weapon was upgraded to primal with recipe #11 of the cube – it has a red pentagram in the bottom right corner.
Hopefully, this will prevent some misunderstanding in the future
