I’m really baffled by this. I googled it and read a ton of posts saying the Broken Crown wasn’t working correctly and almost every time the op just didn’t understand the mechanic.
That said, I’ve tried it while my character was wearing the BC, while my follower was wearing it, while both of us were wearing it, and even tried it as one of my cube items. I also tried different types of gems. I have not even once seen any extra gems drop.
Are you talking about the crowns on various characters in your profile?
Because that’s Leoric’s Crown… not Broken Crown.
Looking at my profile won’t show it because I took it off since it’s not working. I was using it on my 70 seasonal necro.
From my experience, the Broken Crown is working as intended.
If your Follower is wearing it, the extra Gems that drop will be the same as those in YOUR Character’s Helm. Not your Follower’s Helm. Obviously, if you’re wearing it, the extra Gems that drop will also be the same as those in your Character’s Helm. Even if you and your Follower are wearing it, the extra Gems you get will still be the same as those in your Character’s Helm.
The extra Gems that drop will be at your Character’s feet. So, if you’re playing a ranged Character and you kill monsters at the edge of your screen, don’t run off too quick before checking the ground. Otherwise you might miss them.
The second Boss Bounty should give you twice the Gems you got the first time. And all the Gems that drop will be near the Chest when you open it.
I did that exact test and got zero extra gem drops.
Can you post a screenshot of the Broken Crown you’re equipping? (Since you don’t currently have it equipped).
Another thing to consider is that when the gems drop, it is from your position. But if you’re zipping around quickly, due to latency, sometimes the gems drop at a prior position you’re standing at so it will feel like you’re not dropping gems.
I personally haven’t seen any issues with BC not working.

Working fine for me today. When playing solo, my follower had it equipped. When I ran with my friend, I put it in the cube.
Couple of things that can cause people problems with this are:
If you do not have a gem in your characters hat, a second gem will not drop and
The second gem will be always be the same type as the one in your hat, not your followers.

Actually, IMHO, the extra gems from Broken Crown drop first, kinda on your toon, and the original ones drop where they drop.
Didn’t notice Broken Crown not working.
However, if you wanna go for gems, important places I always wanna have it active:

BC works perfectly for me, as others have pointed out. Doesn’t matter if you wear it, your follower or if it’s one of the 3 (4) cube powers. But your char needs to have a gem in their headpiece, because that one will be replicated. And the extra gems drop first. And and your char’s feet.
And having the power more than once will NOT drop additional gems.
Something to bear in mind is that the extra gems will match the quality of what drops, not what is worn.
So in other words, if the game triggers a Marquis Emerald to drop, and you’re wearing a Flawless Royal Topaz, you’ll get TWO Marquis drops - one emerald, one topaz. In the same scenario if you were instead wearing an Imperial Emerald, you’d still get TWO Marquis drops - two emeralds.

I completely uninstalled and reinstalled the game and the extra gem drops are working correctly now.
