On both seasonal journeys (EU and NA) I got Boon of the Hoarder as drops from a rift guardian without ever having been in a vault in the season.
I know it was introduced as drop long ago, but only if it was obtained from Greed before and then lost. Was this change announced somewhere or is it a bug?
I don’t know about that, but i always get BoH from RGs, if i haven’t got it from Greed allready.
There’s a small bug that if you visit Not the Cow level, it flags you as if you have visited the Vault. That’ll make the Hoarder drop from a GR prematurely.

Strangely enough, I had gotten my boon of the hoarder gem without visiting either the cow level or vault this season. I had simply assumed that it was normal, but I suppose not.
The other possibility is that opening the Echoing Nightmare does this as well.
Got mine from a RG, was abit surprised as that never happend before (from memory), was a nice surprise though 
I’ve always gotten mine from a vault, but good to know.
It definitely drops from Greed, but if you are doing GRs and have all the other legendary gems, you will get boon.
Ah - that was it probably. IIRC I got goblin spawned cow levels in both seasons.
