From what I read on guides the black mushroom drops in Cathedral Level 1 and requires a specific corner square room that has 2 entrances…I’ve seen that specific room a few times and every time there’s no mushroom…is it not a guaranteed drop with that room?

Sometimes that room layout the guides pointed out (two entrances) will spawn near a spiral staircase downwards which will NOT have the Black Mushroom.
It took me about 15-20 tries before I found it.
Which was more tries than getting the Gibbering Gemstone!
No, it is not a guarantee. Like most everything else it is hit or miss.
so the room can spawn but the mushroom may not necessarily spawn?
It is NOT a guaranteed drop. Just like that other items you need for the staff.

The hilarious bit about that is, I spent a good 6 months looking on the wrong level before I realized my mistake
