I selected her to show an example of a figure in the distant past who was transgender. Whatever controversies she was involved in are irrelevant; the point is that transgenderism is not a modern construct.
It isn’t only Christianity that opposed homosexuality, but Christianity was the dominant religion practiced in medieval Europe, and the churches often had the political power to enforce their religion’s code onto society.
The Crusaders in Diablo certainly feature modified Christian iconography, but they’re notably different from Christianity in many respects. The crusaders, for example, dont try to convert people with missionaries like many Christians did in the past, and they certainly don’t go around murdering civilians like the real crusaders. My point here, is that while the crusaders do seem loosely based around Christianity, they have several notable differences. It seems like quite a stretch to assume their religion is hostile to homosexuality based just on that loose association.

We do not know how Westmarch treats homosexuals, and that is the point: we do not want to. But the fact that they are Christianity-inspired defeats your argument that homosexuality can fit into Diablo’s universe. In fact, very few cultures have tolerated open homosexuality.
The emperor you mentioned was a failure and a disgrace. He was not “transgender”, he was a demented young man who came to ruin. He was certainly not accepted. Your idea of “transgender” is an absolutely modern construct.

The province of your character and their story is the province of your own mind and creative license. Enjoy it to your furthest extent, just don’t demand I participate in it. (That means go be or imagine whatever story lines and dramas you want, they’re your imaginings, but don’t try to control my ability to do the same)
There are more characters in the Diablo world than our own characters however.
Also, even our own characters might have a backstory written by Blizzard. In D3 they did. In D4 it seems like they might not.

Blizzard could add the Rainbow LGBT Banner to the game to allow players to customize their banner as such for representation. With this, we can consider this matter closed and move on.
I don’t see why not. The canon version of our classes in Diablo 3 aren’t necessarily the same characters that we play in Diablo 3. For example, you could play as a male wizard (whom named could be almost whatever you want it to be), but canonically the wizard is a female named Li-Ming. As such, the same could be done about Diablo 4. For example, the canon version of the rogue for Diablo 4 could be the same that we saw in the trailer, while the one we play could be a different person.

Blizzard writes the lore. I seriously doubt they would make a universe openly hostile to gay people, who they hope to sell to, unless they were going to make a plot about a person struggling against that hostility. I’ve already outlined why I don’t think its reasonable to assume that the priests of Zakarum are homophobic just because their religion is loosely tied to Christianity in the real world.
Elagabalus was a sad case of a child given far more power than you can reasonably expect a child to handle. I view it in a tragic light. Regardless, that doesn’t really have an impact on whether or not we can consider them an early example of what we might today call a transgender person.
Even if you don’t consider Elagabalus a valid example, accounts of things like this go back as far as recorded history. Kalonymus ben Kalonymus gave an account of gender dysphoria in the 1400s, in what is now France. Numerous accounts of trans men were also recorded throughout europe in the middle ages. There’s a Wikipedia page on transgender history that goes through plenty of examples like this, without even looking beyond the overview.
Its true that the modern concept of transgender hasn’t always existed. But people who feel they were born into the wrong sex are by no means a new phenomenon.
