Gamble at Kadala, or upgrade rare rings in Kanai’s Cube also.
It’s a random drop, so anywhere random drops occur (as long as you’re playing at T1 or higher), will give you a chance of getting it.
Alternatively, gamble w/ Kadala, upgrade rares, reforge non-ancient puzzle rings.

Yes, random. For some reason though, for me, I’ve had three Puzzle Rings drop. All three were Ancient.
I used one. Keeping one for the Altar. Will use the other with my wife when she’s ready.

I’ve seen one ancient puzzle so far, of a total of 15-20.
My necromancer found a fourth Puzzle Ring, also Ancient.
I then started a barbarian after not getting a Ramaladni and he found a Puzzle Ring… but it was just a standard one. So, it appears the “streak” is over. Or not. I’ve not gone back to the necromancer yet but I will soon enough to get more shards for the barbie to spend.
I did this after farming and farming so then my very next gr I got an ancient now I need 2 primals lol RNG can be a harsh mistress someties
Just keep spamming, its a numbers game, you’ll get them. There’s a reason why I’ve lost count of how many primals I’ve gotten and why people complain they’ve gotten none.
You could always reforge regular puzzle rings hoping to get an ancient?
Does upgrading yellow ring at toon’s level 29 still have better chance to get puzzle ring?
You cannot upgrade a level 29 item. Only level 70 rares can be upgraded in the cube. Also, only level 70 items can be ancient.
you’re right, it was from ethereal season and the level is 31 to get the puzzle ring efficiently, and yeah, ring level almost matches to the character level,.

Upgrading rare rings must be a desperate measure if nothing else works. There are 24-28 different ring outcomes that you can acquire from rare item upgrading depending on the class. That’s less than 4% chance with another 1/10 rarity modifier for a ring you want to be ancient quality on top of it. That simply means you have around 3.5 to 4.1 in a thousand chance (0.357-0.416%) to roll an ancient ring you want and this is not considering item weights. That’s not impossible, but still require a hefty amount if you don’t share someone else’s luck.
Loot pool of rings is large enough for you to waste a good chunk of materials to get the right one; and yet you still expect the right item to be an ancient, with 1/10 rarity rate. Bigger the potential loot pool, dimmer your chances of getting item you want with high quality from anywhere.
Now remember, crafting materials are harder to replace but shards can be refreshed easily by hitting up rifts. While Kadala also has another filter for handing out legendaries, her pity rate can be improved thanks to Altar node that doubles the legendary rate from Kadala.
You also need those Forgotten Souls to unlock the next node and that sacrifice also require Horadric materials, so mind your reforges. Just gamble from Kadala and hit up Nephalem and GR90 as quick as you can. Clear a few rifts then hit up Kadala to dump everything on rings. Eventually you’ll get one.
Actually…roll 20 rare rings and upgrade them. I got 3 puzzle rings 2 were ancient. Recipe #3 is not to be overlooked.
I did exactly this:
Gamble for the ring, then upgrade at kanai cube.
Reforge, number 2 on the cube.
Didn’t using the new recipe also work? Upgrade a regular one to a Primal version then sacrifice that?
Just saying, it might be an option.
I did lots of GR, got several regular ones so I decided to reforge. Took me about 5-6 tries and I got an ancient.
… then the next one that dropped was ancient.
