Rarest unique is technically tyraels might… think nihl pindle and baal are the only ones that can drop it.

But rarest items to me are rares with the most sought after combination of mods. Like a rare ethereal berserker axe with up to 300% ED, 40% IAS, 2 open sockets, Fools mod and self repair…

Basicly perfectly rolled rare items with all desirable stats.
