I always though the charm had to be dropped by Baal himself but, in a game yesterday, someone got one from his minions at the throne asked if it counts as a Baal charm and a couple people said yes.
Do charms dropped from his minions at the throne count as “Baal charms”?
Its a Ilvl 99 Grand Charm, which mean it can roll the absolute highest modifiers (say +45 Life+x to skill GCs). People keep them because you can reroll them indefinitely using the cube until you get something you want.
Its only Hell Baal that drops them though.

Those people don’t know what they’re talking about. A “Baal Grand Charm” means a Grand Charm with ilvl 99 and only Baal can drop one.
The second highest are Diablo and Nihlathak and they are both level 95. Baal’s Minions are 85+3 so that charm would be ilvl88.
Edit: Actually Diablo is Level 94 and Nihlathak is level 95.
I would like to add that Diablo and Nihlathak Grand Charms can also roll +41-45 Life. The prefix requires an ilvl of 91.
