I’ve tried a couple different magic helms, a magic jewel, ral rune, and perfect ruby, and no success creating a blood helm. Is this a known issue?
Are you sure the base item is one of the following?
Helm (normal version)
Casque (exceptional version)
Armet (elite version)
And the helm is indeed magic as in the text is blue?

Yep, they are blue text magic helms. At a total loss. I tied a blue text and yellow as well just to try different things
Helm is a generic term, I know, but there is also a specific helmet called Helm. Check the link above and look for Helm and make sure that is what you are using. If so, then I will try and see if I can reproduce. 
Oh I see, I am using a grim helm and a great helm, those are incompatible?
Correct. The Base Item has to be a Helm, Casque or Armet.
