Why do I feel like Ive never seen this item before
Because the average player rushes to higher difficulties, where it becomes harder to find. It’s the same reason why many players don’t find The Hand of Broc and other very low level unique items…
It is like Biggon’s Bonnet. So low level that you almost never find it.

Haha, go for it! It is not half bad for a very low level item, though probably not as good as an actual unique Shako lol.
Usually find these low level items doing some NM meph runs, I always save certain ones for the next builds.
The odds of them dropping throughout normal during your questing, is far too low IMO, not that i want to make this about drop rates, just the thrill of a low set or unique along the questing, really is a great experience.

Item drop formula starts at the top of the available treasure classes and works its way down. Every item belongs to a treasure class, increasing in number based on ascending quality (running up through the base items in their normal then exceptional and finally elite versions, with set and unique further complicating things).
When you’re running around in, say, Hell, in an area that can drop every item, it has 87 treasure classes to work through to get to the bottom. Low-level uniques like Lenymo are pretty much dead last (treasure class #3), so the game has to roll the dice and fail for every treasure class starting at the top before it gets to TC3. The odds of this happening are very low (think about how many sashes you find after you’ve left normal act 4).
Further complicating this, sets and uniques have another marker called quality level that bumps up the minimum monster level that can drop them. Lenymo is qlvl 10, so you simply cannot get it running around in the Blood Moor, for example. You’d have to get to somewhere like at least Jail Level 1 to where the monster level is high enough to drop Lenymo, but now those monsters can drop from up to TC9 (light gloves, heavy belts, that sort of thing) and have to fail those drop check rolls before they get down to TC3 where sashes can drop.
In short, the more stuff a monster can drop, the less likely it is to drop super low level stuff, not just because they can drop more stuff but also because they’re more likely to drop higher level stuff than lower level stuff.

Lenymo I’ve found. Same with Biggin’s Bonnet. Same with most every low unique (in fact, I’ve found most every item at this point, think I’m at 96% grail completion with my relatively casual play). And while I haven’t necessarily farmed for it, one of those low level items eludes me. The Hand of Broc. Figure I’ll save it for after I find every other item, just for the lols of having such a low level item be my last item needed for grail.

Yes the high TC’s have an incredible high chance to fail, and are very rare because of that. The lower TC’s are very rare too fail, mean the highest of them drop very often, while making the even lower TC’s more rare. Leading to the lowest TC’s beeing rare again …
