The main problem comes from that ppl dont care what the ui tells them. You would have right IF after you click on the x the game would not ask for delete confirmation.
Well this “off META” ui alone proves how spoiled are the gamers today and how fast they fail when they need to use their brain.
You are ignorant. That is everyone, in every software system ever. Read a book on UX and educate yourself before you come on here and spout your false, uninformed takes.
Whether or not this is correct it is still just bad UI design. A large portion of UI design is to try and “idiot proof” the system as much as possible for the users to not do catastrophic damage to a system.

Who cares what every other system use? If you are lazy to read the tutorial and also lazy to read a pop ui element of delete confirmation then you deserved to lose your hero. Yep doctor tells you what to do or what is going to happen. Ui also makes the same. Lazy ppl just dont care. The ui (doctor) is there but you just dont listen.

Humans make mistakes.
Humans tired from playing D2 until 4 am in the morning make even more mistakes.
The process of deleting characters needs to be mistake proofed.
P L E N T Y of examples from other games to draw on that would greatly reduce the heartache that comes from deleting a beloved character.

I cannot recall a single time an exit to any game has been in the bottom right-hand corner, especially considering there’s a button on that left that reads “Exit Diablo 2.”
Tell me why there’d be two exits on the same screen.
When you mouse over it, it says “Delete” and I’m also pretty sure it asks for confirmation. 2 button clicks.
And this is why products need warning labels.

Do you want to champion cars without seat belts because people need to know how to drive?
Do you want to endorse buildings without exit signs, because anyone dumb enough not to know how to get out should die in the fire?
Some kid last night deleted a lvl 86. It was his fault. No doubt. If he had been asked to type “delete” he wouldn’t have made such a bone head mistake.

If you drink to much (what was your choice just like playing to much) then a car going to look what your imagination wants it to be. It a perfect analogy for pathetic excuses like " i was toooo tired to watch whats the ui is telling me".

There are seat belts. Hover over x its says delete. Click on x its asks “are you sure”. You want more seat belts for 1 person.
There are exit signs. There is exit diablo button.
Its not a mistake to click on confirmation. At best its lazyness.
