Ive seen posts where this happens to others. I’ll be firing off blizzard and then suddenly i’m teleporting! It just randomly rotates thru my bound skills on my right click only. there a fix or just a “your stuck with it thing”?
Mouse wheel perhaps?

i have this as well.. my wwsin sometimes wormholes through the map similar to ruberbanding though i never was in that spot i got wormholed to, also skillbar skills tend to do weird stuff, like i use ww as hard skill on right mouse buttonand i’ve literally notied ww changing back to normal attack when sitting idly in town.. ( my cat came for cuddles, so i had to make a small break, tho was still sitting in front of the monitor, and noticed this)
It could be related to that. maybe equipment wiggle or something, but it does happen and i am definitely not touching the mouse wheel.
This is a problem caused by the “new” hotkey function implemented to D2R. If you press hotkeys and right mouse button too fast, it may happen a hotkey swaps to your right mouse skill.
It is, in fact, a very poor implementation: Pressing the hotkey swaps your right mouse button once, executes a click and immediately swaps back to your standard skill. If you “interrupt” this process, standard skill may get swapped permanently by your last used hotkey, in this case: Teleport.
It is just a matter of lazy programming.
There’s no way to solve this except by playing with a controller or using the “old” method of swapping between skills or being very careful with using hotkeys.
there are only 2 possible explanations for this
neither of which have your skill being reassigned
both of these scenarios have been covered by previous posts but i’ll go into more detail
the first of which is where your right skill icon does not change
but you are teleporting to random location you did not click
this would be from an equipped " the occulus" orb which has a 25% chance to teleport you when your hit
the 2nd option is you are pressing a button that scrolls through your equipped right mouse skills. by default this is the mouse wheel scrolling up and down. but it can be bound to other things. its possible you using the mouse wheel accidentally.. possibly because u bound a middle mouse click to a skill. or you bound the scrolling through skills to something that you’re unintentionally pressing

oh i have seen that happen before.
or rather i noticed the skill fails when i press the hotkey because for some reason it got moved to the right mouse click.
normally when summoning my wolves/ bears in town though. not ever in-combat
i was unable to ever determine a root cause though
I did go into controls settings and changed mouse wheel bindings to other keys (that i never use). nothing bound to the mousewheel now. going to try that for a while and see if it helps.
I even went into regedit and turned off the mouse wheel there too just to cover all the bases.
Now we wait….. 
I’ve seen the mouse wheel be screwy sometimes where the “scroll direction” is opposite, so scrolling the mouse one way will scroll through the hoykey’d skills in the opposite to expected order, then it fixes itself and scrolls in the expected order. Often happens on my paladin scrolling between auras, after just joining a game, then works as intended the rest of the game.
It is done within the confines of how Diablo II was originally designed. This feature was designed as a QOL improvement, but it does so within the limitaitons of the game engine.
ok, its been at least 2 weeks since i removed all skills from mouse wheel and also have nothing mapped to the mouse wheel. Issue of random skill rotation has not happened since.
