I’ve compared controller use FPS vs mouse.
I noticed FPS while using the controller drops immensely. Using dynamic scaling I get 100 frames, but as soon as the controller is connected, my FPS drops to 55 and hovers around 60, with an average of 59.
Can we look into this and get this addressed? I’d like to use the controller and kick back in my chair while in a party with friends. Thank you.

In the beta the framerate would lock to 24fps when viewing your inventory. Does the same thing still happen?
No it doesn’t appear to happen. The frame rate is locking to 60 though with controller but its weird because sometimes the frame rate drops even lower.
I have the exact same issue. I have a very powerful PC and get 175 fps with mouse and keyboard on max settings.
BUT the moment I use my xbox one controller and the UI flashes/swaps to controller UI, its like the entire scene is locked to 60 hz with terrible stutter during motion instead of my usual 240hz. With mouse and keyboard everything is fine.

i9-9900K@5.0ghz
32gigs ram
I had controller problems too swapping between controller UI and mouse UI.
SOLVED: Play D2R through steam as non steam game and the controller will work flawlessly. I am running the direct exe file not the launcher. Hope this helps.

CONFIRMED THIS WORKS!
As long as you have checked off generic controller compatibility in the steam>settings>controller section, and make sure you choose D2R.exe directly and not it’s launcher, and add it via non-steam game, upon launching I am getting my proper 175+ fps in game with controller now! Thanks a bunch for this suggestion!
Clearly the 3rd party support steam offers for controllers is less buggy than the current D2R implementation. I also ran launch argument -window-mode exclusive through blizzard launcher for D2R and the exclusive seems to be working, adding more fps/smoothness it seems.

I’m unsure if this is a bug, but when i start the .exe from steam the game cannot connect to the servers.
Anyone else getting this?
I experienced this problem but can confirm that if you turn off vibration in the D2R game play menu (under controller) it fixes the issue.

thanks a lot man, u saved my life!! but you made a little mistake in your explanation, you need to check the box for compatibility with xbox controller and surprise…I got now 120 fps with my xbox controller. make sure you change the path in the game setting in steam, in order to lauch the game directly from steam with the D2R.exe, by default it will launch the launcher
