Just got to Act IV. Cleared the entire Outer Steppes, explored the whole map and found no exit that takes me to Plains of Despair.

Probably missed it, try going near the edges and turning the map off.
I went around the entire perimeter 3 times to be sure. I ended up leaving the game, made a new one and now it’s there.
