I don’t know what is going on but a couple games in a row and Andariel doesn’t drop a single item on Hell. A friend of mine noticed the same thing. What is up with that?
I noticed she doesn’t drop if she hasn’t been on screen during the fight. After killing her off screen once I now make sure she’s in view when she dies. Not sure if this might be what’s happening?
Same for me, can’t remember if it was like that in D2LOD but sometimes she doesn’t drop anything or only 1 item.

When you are solo, then there is still a no drop chance, that is depending on numbers of Players ingame. When the game has at least 3 Players, then the there is no 0 drop chance and she will drop normally.

Depends where you have killed her, some items may have fallen into the cracks on the ground.
The game doesn’t roll dices, it actually use algorithms for random generation. Can you stop using the word dice, it’s annoyingly stupid.
